Forms and Other Documents for Consumers

The following Office of Consumer Protection documents are available to download and print. Forms cannot be submitted online. Please follow any instructions carefully. See Help for assistance with viewing Portable Document Format (PDF) files.

Print-only forms – Most forms must be printed, filled out manually and delivered or mailed to the address indicated on the form.

Fill-and-print forms – Some forms contain form fields that can be filled in on-screen. You may fill the form in on the computer and print the completed form. Be sure to sign it before delivering or mailing it to the address indicated on the form.

Advance Health Care Directives

This form may be used to indicate a consumer's health care choices should he or she become terminally ill and unable to express decisions on whether to accept or refuse medical treatment. (This information can be provided in other formats, provided the document meets the requirements specified in MCA 50-9-103.)

Note: To file a directive in the Montana End-of-Life Registry, it must be accompanied by the Consumer Registration Agreement that follows.
